- Gather background data, review referrals, select and modify evaluation tools, and evaluate patients determining the level of our patient's physical and psychosocial functioning.
- Develop and revise the plan of care in consultation with the physician and other care-team members.
- Responsible for planning, evaluation, and treatment implementation of assigned caseload.
- Preparing clinical notes.
- Evaluating outcomes.
- Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.

Alta Health Services is excited to announce a PRN Physical Therapist position for our home health division in beautiful Salt Lake City, UT.

There are no 15 minute encounters, we do not schedule at 125% production, and we encourage our therapists to spend the time to ensure each patient receives the care they require.
Our Physical Therapist will help our patients with the following:
Our Physical Therapist will enjoy a competitive hourly rate of $50-$55 as well as a per visit rate ranging between $70-$78.
