    About the Role:

    As a Discharge Planner, you will play a vital role in bridging the gap between inpatient care and community-based services. Your primary responsibility will be to ensure seamless transitions for patients as they leave inpatient facilities and enter our care. This requires close collaboration with healthcare teams across various settings, including emergency departments and acute care units.

    Your Key Responsibilities:
    • Represent our organization in inpatient and emergency settings to coordinate patient transfers
    • Conduct thorough patient screenings to identify needs and develop personalized transition plans
    • Advocate for patients' access to necessary health and social services
    • Collaborate with multidisciplinary teams to establish trust and rapport with patients, families, and staff
