- Completes psychosocial assessment, Plan of Action, and Plan of Action updates within established timeframe.
- Initiate (upon admission) discharge planning in collaboration with patient and treatment team.
- Demonstrate skill of writing individualized, behavioral, and measurable treatment goals based upon information from psychosocial assessment and psychiatric evaluation.
- Conduct daily group therapy sessions.
- Conduct individual and/or family therapy sessions as defined by treatment plan.
- Collaborate with psychiatrist to ensure coordination of care.
- Schedule medication management appointments.
- Obtain insurance benefit information and authorization for treatment; perform ongoing utilization reviews.
- Demonstrate skill in documentation to meet payer and regulatory standards.
- File all progress notes in the medical record within 24 hours of Date of Service.
- Respond appropriately to patient's requests.
- Provide comprehensive education, instructions, and/or advocacy information to assist patients and significant others to secure necessary health care follow-up services.
- Provide referring providers and/or therapists with biweekly updates of patients. compliance with and progress in treatment.
- Adhere to policies and procedures.
- Prepare and assist with audits when requested.
- Assist with community and marketing events, as needed.
- Demonstrate ability to work and communicate effectively as a team member with providers and other staff.
- Demonstrate ability to organize and prioritize tasks effectively.
- Performs other duties as assigned.
