    Family Medicine Physician Job Description

    We are seeking a qualified Family Medicine physician to join our team at M Health Fairview Clinic - Andover. Our thriving practice offers multiple on-site specialties and an experienced staff.

    Key Practice Details:

    • 14-provider primary care practice with access to 7+ specialists onsite.
    • Average patient encounters: 18-22 patients per day.
    • 34 patient contact hours per week.
    • Call Frequency: 1:20 low intensity - phone consultations only with first-tier nurse triage.
    • Flexible schedule available: 0.8 to 1.0 FTE (Full Time).
    • Fully integrated Electronic Medical Record (EPIC).

    Requirements

    • Graduate of an accredited ACGME/AOA Family Medicine residency program.
    • Board certification or board eligibility through the American Board of Medical Specialties or American Osteopathic Association.
    • Licensed in Minnesota or eligible for licensure through the Minnesota Board of Medical Practice.
    • Ability to obtain and/or maintain DEA certification for the State of Minnesota.
    • Meet credentialing and privileges requirements.
