Responsible for ensuring evidence-based care is delivered to all patients
Responsibilities:
+ Coordinates the daily operations of the specialized clinical program with a strong focus on quality assurance and improvement + Responsible for working in collaboration with the healthcare team to coordinate the care and service to patients across the continuum of care + Collaborates the plan of care utilizing standardized care protocols in accordance with nationally recognized evidence based care guidelines + Responsible for managing the program including but not limited to community education, prevention, diagnosis, treatment, therapy, and regaining functional status + Assists patients in understanding their diagnosis, treatment options and the resources available + A wide degree of creativity and latitude is expected + May lead and direct the work of others + Performs other related duties as assigned + Familiar with standard concepts, practices, and procedures within the field + Regular and predictable attendance is an essential job function + Competent to meet age specific needs of the unit assigned Requirements and
Minimum Qualifications:
+ BSN required + State of Idaho Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the 'multistate privilege to practice' compact with Idaho required + Previous experience in program development + Clinical knowledge base and data management skills + Knowledge of regulatory standards, evidence-based guidelines + Strong organizational and communication skills
