Job summary
The Licensed Vocational Nurse ECM Case Manager plays a critical role in supporting members with complex medical and behavioral health needs through direct care, care coordination, and member engagement.This position works in alignment with Enhanced Care Management requirements and contracted services to promote improved health outcomes, continuity of care, and member empowerment.
Qualifications
- Active and unrestricted Licensed Vocational Nurse (LVN) license in the State of California
- One to two years of clinical nursing experience preferred
