- Practice Setting: Outpatient, Telehealth
- Type of cases and required procedures: Interdisciplinary Team consultations, medication consults, telehealth visits, Care Transition Interventions, admissions, and symptom management during call
- Credentialing timeframe: 0–10 days
- Electronic Medical Record (EMR): Epic
- Certifications required: Must be Board Certified or Board Eligible within 5 years of residency in Family Medicine (FM) or Internal Medicine (IM), Basic Life Support (BLS)
