    Essential Functions-Management:

    • Create an environment that is consistent with the company's mission and vision by creating a positive morale to maximize team member job satisfaction and optimize retention.
    • Manage all employee relations issues and ensure compliance with Human Resource Policy and Procedures; seek assistance of Human Resources when needed.
    • Assist in developing fiscal budgets for center.
    • Assist in the development and implementation of center-specific strategic plan, including marketing/sales plans for referrals. Responsible for achieving the plan; overseeing center staff participation in plan to ensure success.
    • Ensure strong physician and payer relations, coordinate implementation of clinical specialty programs as requested by the customer.
    • Maintain responsibility for ensuring center profitability. Interprets financial statements and performance metrics to develops strategic plans and address underperforming areas.
    • Manage Aide staff and hold them accountable for delivering highest quality customer service and scheduling of patients.
    • Ensure clinical team delivers the highest quality clinical care. Provide patient care skills development opportunities for clinical staff as appropriate. Recommend staff for recognition.
    • Ensure efficient caseload management using available reports (e.g. 1-2-3, Active vs. Scheduled, etc.)
    • Ensure appropriate productivity and staffing levels working with market leader (Lead Aide, MCO or COD), using our productivity matrix tool.
    • Conduct orientation and training of staff to implement VibrantCare clinical programs and operational objectives.
    • Identify open positions and hire clinical staff in conjunction with MCO/COD. Organize daily work schedule to maintain optimal patient care.
    • Conduct accurate and timely Performance Planning & Reviews (PP&R) of center staff with specific focus on achievable development goals.
    • Timely approval and management of Personal Time Off (PTO) requests, bi-weekly payroll/time sheets for members of assigned team.
    • Ensure general appearance and cleanliness of the clinic at all times.
    • Attend and actively participate in CM/COD bi-weekly meeting with completed agenda.
    • Approve all order requests (ARS & Staples), ensuring effective cost management

    Essential Functions-Compliance:

    • Ensure VibrantCare Rehab Agency, State, Federal and local regulations, policies, procedures and patient care standards are met.
    • Maintain employee files with current CPR certifications, current license (when applicable), TB or any required vaccination(s).
    • Participate in annual Patient Care Committee and sub-committee meetings.
    • Conduct monthly staff meetings to educate team members on company news, clinic performance, improve operational issues including mandatory topics as communicated by market COD or MCO (if applicable) and provide clinical in-services.
    Ensure compliance with all Medicare Rehab Agency Conditions of Participation including:

    • Review and maintain the VibrantCare Center Facts Manual with all current information.
    • Review the VibrantCare Clinical Operations Policies & Procedures Manual on a yearly basis and adhere to the policies and procedures set forth.
    • Participate in the

    Quality Improvement Program:
    execute quarterly quality assurance audits and develop action plans to address non-compliant indicator scores.

    • Complete all Medicare compliance annual trainings as required by regulations and/or VibrantCare policies.
    • Ensure timely and accurate completion of all clinic cleaning & maintenance logs (including pool cleaning binder if applicable)
    • Deliver/participate in all Emergency Preparedness Program (EPP) drills and activities.
    • Ensure completion of annual fire inspections, fire extinguisher maintenance and obtain proof of annual fire alarm and sprinkler inspection.
    • Ensure annual equipment calibration.
    • Complete Occupational Safety and Health Administration (OSHA) training on a yearly basis and adhere to standards set forth in OSHA standards and other governmental regulations (e.g. Blood borne pathogens, Hazardous material communication).

    Essential Functions-Treatment:

    • Understand and adhere to all elements of the state Practice Act pertaining to the provision of PT or OT services.
    • Evaluate, treat, and delegate appropriate skilled intervention to patients for whom physical therapy or occupational therapy is medically necessary.
    • Provide thorough and timely documentation in which goals, progress toward functional goals, and medical necessity are clearly stated.
    • Complete all necessary documentation required to obtain pre-certification or extension of treatment.
    • Maintain open and respectful communication with co-workers, physicians, patients, family members, and third party payers at all times.
    • Complete required ongoing continuing education coursework to meet state licensure for the defined reporting period
    • Research unfamiliar diagnoses, procedures as needed

    Additional Responsibilities:

    • Act as a role model for the company and profession; as a resource for peers.
    • Maintain current state licensure
    • Maintain a current CPR certification that meets state licensure requirements
    • Assist in general maintenance duties around the center, to keep the area clean and professional looking.
    • Assist in customer service responsibilities including phone answering, patient registration, scheduling and other general receptionist duties as requested.
    • Utilize time efficiently and effectively to meet productivity requirements of the facility.
    • Maintain current awareness of general changes in the health care field and specific issues impacting the practice of physical therapy.
    • Perform other duties or special projects as assigned

    Required Skills & Abilities:

    • Effective organizational, time management and planning skills.
    • Effective communication skills for accurate, concise and organized oral and written skills for report writing and presentation of information.
    • Ability to demonstrate exceptional judgment with respect to patient status, safety and well-being.
    • Good problem solving skills to identify and prioritize issues in a responsive manner and implement and execute action plan in a timely manner.
    • Basic proficiency in utilization of Windows based operating system (e.g. Word, Excel).
    • Ability to work effectively as a team member.
    • Leadership skills sufficient to effectively direct and supervise the activities of assistants and aides in delivering quality care to patients. Includes coaching and teaching skills.
    • Effective clinical problem solving skills (ex. identify and prioritize issues in responsive manner, develop and execute sound, innovative action plans, ensure appropriate results, etc.).

    Physical Requirements:

    • Ability to transfer or assist patients using safe body mechanics when needed; must be capable of safely lifting fifty (50) pounds of weight alone.
    • Ability to walk, bend, stand, and reach constantly during a work day/shift.
    • Visual acuity (near and distant) sufficient to maintain accurate records, recognize people, and understand written direction.
    • Ability to speak and hear sufficiently to understand and give directions.
    • Ability to participate in sustained activities for many hours in duration in accordance with state labor laws.
    • Ability to push wheeled equipment through the facility and in the community.
    • Possess fine motor skills for legible and accurate writing of reports, charting, scheduling, daily correspondence and presentations, either manually, using keyboard entry or through use of dictation equipment.
    • Possess fine motor skills for effective and efficient handling of diagnostic or therapeutic equipment

    Required Credentials:

    • Graduation from a physical therapist or Occupational Therapy education program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or, in the case of an internationally educated physical therapist, from a non-CAPTE accredited program seeking licensure as a physical therapist, a substantially equivalent education;
    • State licensure/registration where applicable.
