    Position Overview:

    Bluestone Physician Services operates a mobile primary care practice that offers a distinctive and comprehensive care model; tailoring care that is preventive, proactive, and involves all members of the care team, patients, and their families. Join our team as a Nurse Practitioner or Physician Assistant as we provide specialized primary care for elderly patients residing in Assisted Living and Memory Care communities in and around Port Charlotte, FL.

    Our Care Model delivers personalized care that is preventive, proactive, and involves all members of the care team, patients, and their families. Our patients witness a significant reduction in ER visits, hospitalizations, and readmissions compared to patients with similar conditions and complexities.

    Responsibilities:

    • Deliver on-site, collaborative primary care to patients within Assisted Living and Memory Care communities

    • Full-time providers attend to patients Monday-Thursday; the option of Fridays as a clinical or administrative day

    • Oversee a patient panel of 200 individuals (averaging 14-16 patients per day)

    • Ability to create and manage your own schedule

    • Develop and execute tailored care plans based on patients' needs and preferences

    • Receive clinical support from Clinical Assistants and Care Coordination via LPNs, RNs, and Social Workers

    • Access necessary diagnostics like labs and x-rays

    • Collaborate with community partners in areas such as home care and hospice when needed

    Qualifications:

    • Graduate from an accredited Nurse Practitioner Education Program or Physician Assistant Education Program

    • Board Certified/Board Prepared by AANP, ANCC, or NCCPA

    • Licensed or eligible to obtain licensure through the MN Board of Nursing

    • Able to obtain and/or maintain DEA certification for the State of Wisconsin

    • Demonstrated effective communication skills with a strong verbal aptitude

    • Outstanding interpersonal and customer service abilities

    • Must possess a valid driver's license and meet minimum state-required liability auto insurance; local travel is mandatory
