    The Employment Support Provider assists with the day-to-day needs of clients in employment settings. They carry out responsibilities in the following functional areas: client rights, job coaching and regulatory compliance.

    Essential Functions and Desired Outcomes

    + Performs duties relating to the support, care, welfare, supervision and training of persons with mental illness, developmental disabilities, and/or acute, chronic or long-term conditions.

    + Serve as a role model and coach for clients regarding developing and using effective problem solving, decision-making, conflict resolutions, independent living skills and responsibilities.

    + Learn the independent living skills and development needs of clients to assist clients in implementing goals and objectives in their employment setting. Provide training, demonstration, and observation both individually and in group sessions about identified areas. Make accommodations as needed to assure client and staff safety. Provide assistance if client is not able to complete a task.

    + Advocates, enforces, and monitors the protection of client rights to ensure the proper treatment of clients.

    + Monitors and documents client behaviors in the work place.

    + Follows all documentation and reporting policies and procedures for any significant events.

    + Coaches, monitors, and reports clients verbal and physical behaviors to ensure the safety and health of clients and others.

    + Responds promptly and competently in emergency response situations.

    + Ensures continuous information exchange between staff, case managers, guardians, family members, funding agencies, employers and service providers, this is done by phone, email and meetings as required.

    + Provide individual services to the clients that are outlined in their plan, following the authorized hours.

    + Provide support and supervision to persons with disabilities in their community work environment.

    + Provide on-site support and maintain a close working relationship with clients and employers.

    + Possess physical aptitudes and capabilities that will enable performance of the job to include: ability to stoop, stand, pull, push, hear, see, kneel, walk, grasp, reach, talk, and lift at least 35 pounds.

    + Other duties as assigned.

    Qualifications

    + Must possess a high school diploma/equivalent and/or related work/life experience.

    + Must be at least 18 years of age.

    + Must possess a valid WI drivers license, and ability to meet agency driving standards. Must provide proof of auto insurance.

    + Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

    + Knowledge of and ability to intervene appropriately in crisis prevention and de-escalation when necessary.

    + Experience working with people who have physical and mental disabilities. Experience working with individuals with severe and persistent mental illness is preferred.
