Job Description and DutiesThe Department of State Hospitals, Metropolitan State Hospital is currently accepting applications for Clinical Social Worker-Community Integration Coordinator Permanent/Fulltime
Working Title:
Clinical Social Worker-Community Integration Coordinator (Health/Correctional Facility) - Safety.
The Clinical Social Worker (CSW), Community Integration Coordinator works as a liaison between community entities and the interdisciplinary teams, maintaining effective relationships with community agencies in the interests of advocating for the discharge and ongoing treatment needs of patients.
The Community Integration Coordinator will provide consultation and functional assistance for complex discharges and serve as a resource to staff and patients regarding community integration options.
This position will coordinate and facilitate treatment groups and provide training for clinical social workers. Regular data collection, program evaluation and performance improvement recommendations for the department is also a core function.REQUIREMENTS:
Applicants must possess completion of a Master's degree in Social Work program from an accredited school of social work and must be registered with California Board of Behavioral Science (BBS) as either ACSW or LCSW.
DESIRABLE QUALIFICATIONS:
A minimum of 1 year work experience as a Clinical Social Worker in a mental health facility is highly desired.
