The role you'll contribute:

Responsible for continuity of care across the continuum with emphasis on quality patient outcomes and cost-effectiveness through decreased length of stay, and control of resource utilization.

Provides Patient/family instruction, scheduled physician appointments, any additional discharge collaboration/coordination in the hospital's EMR and coordinates activities according to patient's need.

The value you'll bring to the team:

• Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self[1]care, nursing facility, home health, physical / psych facilities.
• Acts as liaison with physician to assure appropriate and complete documentation
• Prioritizes activities and completes work assignments on a timely basis while functioning with minimal supervision
• Assists physicians by providing information, suggestions, alternatives, and timely referrals for discharge planning
• Provides continuity of care by coordinating the flow of information between hospital departments and outside organizations
• Utilizes nursing data base to initiate discharge planning by procuring appropriate level of care at discharge, i.e. self[1]care, nursing facility, home health, physical / psych facilities
• Reviews patient medical records for PI / Risk Management per established criteria and provides appropriate documentation of adverse patient occurrences

Qualifications


EDUCATION AND EXPERIENCE REQUIRED:

• High school diploma or (GED) equivalent is required.
• Maintains professional LPN license in the state of Florida.
• 3 years LPN acute care experience

EDUCATION AND EXPERIENCE PREFERRED:

• Five years clinical experience
