    Description

    Job Title: Licensed Practical Nurse - LPN Visit Nurse
    Company: Home Health Care, Inc.
    Schedule: Full-Time or Part-Time Availability

    Compensation: $32 - $33 per visit

    Home Care is in high demand and we are looking for experienced and caring LPNs to join our team at Home Health Care Incorporated Successful applicants will have a proven track record as a LPN, with home care experience a huge benefit but not required. We encourage you to apply today

    Licensed Practical Nurse - LPN Visit Nurse Overview:

  • Provide patient cares according to client's care plan and needs
  • Perform wound care treatments and and assist clients with medication management
  • Teach the client and/or family members about cares and support skills
  • Communicate regularly with RN Case Managers, Supervisors, and the Care Team

    • Licensed Practical Nurse - LPN Visit Nurse Qualifications:

  • Licensed as a Practical Nurse / LPN in the state of Minnesota
  • Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License
  • Demonstrated ability to work autonomously
  • Prior Nursing experience in a TCU, Hospice, or Home Health setting
  • Experience with Electronic Medical Records (EMR) software is preferred
