Location: Nightingale at Stonegate – Skilled Nursing Facility

Status: Full-Time

Position Summary

Nightingale at Stonegate is seeking dedicated and compassionate Certified Nursing Assistants (CNAs) to provide highquality daily care and support to our residents. In this role, you will deliver handson assistance in accordance with each resident's individualized care plan while promoting dignity, comfort, and safety. CNAs are essential members of our care team and play a vital role in creating a positive, residentcentered environment.

Delegation of Authority

As a CNA, you are entrusted with the responsibility, accountability, and authority necessary to carry out your assigned duties in alignment with facility policies, resident care plans, and the direction of licensed nursing staff.

Essential Job Functions

The following duties represent the core responsibilities of this position. Additional tasks may be assigned as needed to support resident care and facility operations.

Key Responsibilities

Administrative & Documentation

• Verify resident identity using wristbands or photo cards before providing care, treatments, or meals.
• Document care accurately and thoroughly on flow sheets, charts, and notes.
• Use only approved facility abbreviations when recording information.
• Promptly report changes in resident condition to the Charge Nurse or Nurse Supervisor.
• Report all accidents, incidents, or unusual observations immediately.

Admission, Transfer & Discharge Support

• Prepare resident rooms for new admissions, ensuring cleanliness and readiness (bed made, name tags placed, admission kit available).
• Greet and escort new residents to their rooms, helping them feel welcomed and supported.
• Introduce residents to roommates, staff, and other residents as appropriate.
• Assist residents in getting settled—providing water, helping them to bed, and ensuring comfort.
• Inventory, label, and document personal belongings according to facility procedures.
