    What You Should Know About the Clinical Pharmacist Role:

    • This is a PRN position.
    • Schedule will include Monday-Friday with hours of 8:30am-5:00pm
    • We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed.

    Job Summary:

    Provides complete pharmaceutical care to each Ohio's Hospice patient by assessing medication profiles, counseling and educating hospice staff, patients, and caregivers regarding medication therapy, and promote the safe use of medication.

    Qualifications:

    • Doctor of Pharmacy (Pharm.D.) or Bachelor of Science (B.S.) degree from an accredited college of pharmacy program. Prior hospital experience preferred.
    • Current license as a Registered Pharmacist, State of Ohio.
    • Valid Driver's License (Safe Driving Record)
    • State Minimum Auto Coverage
    • Ability to complete routine pharmaceutical calculations to accurately compound pharmaceuticals,
    • Competent to add, subtract, multiply, and divide in all units of measure including the metric system, whole numbers, common fractions, and decimals.
    • Competent in Information Technologies
