In-Home Support Specialist

General Description:

Under the general supervision of the Program Director, provide in-home support to caregivers qualifying for Assisted Care Services, that are unable to locate an assisted care provider in their area.

Provide in-home support to children experiencing behavioral needs and, or medical issues that require intensive supervision. Assist the caregiver in and outside of their home. Help families identify natural supports.


Assisted Care Case Manager
The Assisted Care Case Manager will provide in-home support to foster parents and children qualifying for Assisted Care Services.

The Case Manager will provide in-home support to children needing assistance with daily living needs, those who exhibit behavioral needs and, or medical issues that require additional supervision.

The Case Manager will work in collaboration with the caregiver to identify natural and community supports. This is a remote position with required travel within the identified service area. Mileage is reimbursed.

The service area includes Arenac, Bay, Clare, Clinton, Eaton, Genesee, Gladwin, Gratiot, Huron, Ingham, Isabella, Lapeer, Midland, Saginaw, Sanilac, Shiawassee St.

Clair, and Tuscola counties.


Reporting Relationships:
This position reports directly to the Team Supervisor.
