    About PACE

    Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health ), is a new non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

    Position Summary

    The Gary and Mary West PACE (GMWP) Mobile Registered Nurse is responsible for coordinating and providing direct and indirect nursing care to participants at the center as well as at participant's home, working in close coordination with the PACE Physician, RCFE facilities, clinic staff and other interdisciplinary team (IDT) members to manage ongoing care, complex clinical situations, and to support the operation of the GMWP primary care clinic. The Mobile Registered Nurse also, collaborates effectively with colleagues and stakeholders to promote the GMWP values, team culture and mission.

    Essential Functions

    Conduct Nursing assessments to determine the nursing care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans

    • Visit participant's homes to deliver the nursing intervention. This is one of an integral part of the job.
    • Deliver and document nursing interventions as agreed upon in the participants' care plans including but not limited to maintaining a healthy and safe environment, promptly and accurately responding to physician orders, and correctly administering medications and performing ordered tests.
    • Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants' condition or medical.
    • Work with the PACE Physician and other members of the IDT to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home,).
    • Instruct the participant, family and caregivers regarding the disease process, self- care techniques, and prevention.
    • Ensures PACE plan of care is communicated and adhered to by the contracted RCFEs where PACE participants reside through regularly established joint conference meetings.
    • Provides or coordinates PACE or Participant-related education to the contracted RCFEs.
    • Acts as the liaison between PACE and contracted RCFE facilities to assure all appropriate and essential services are implemented timely and efficiently for PACE participants.
    • Oversees the coordination of specialty appointment, Day Center, and other scheduling needs for the participants residing in RCFEs.
    • Conduct regular visits (as appropriate) to contracted RCFEs where PACE participants are residing to facilitate care-coordination.
    • Oversee all direct wound care activities to assure practices are consistent.
    • Identify, plan, implement, and evaluate skin care in participants.
    • Manage and treat specific skin disorders.
    • Coordinates weekly skin rounds.
    • Perform assessment evaluations using techniques including observation and inspection.
    • Confer with the Interdisciplinary Team in the development of the care plan.
    • Provide treatment for skin disorders within established wound care protocols.
    • Monitor the use of preventive measures such as inspection, turning and positioning.
    • Precept new nurses and demonstrate individual procedures to new nursing personnel.
    • Order and tracks the usage of supplies/equipment in prevention and treatment.
    • Assist in modifying the treatment regimen to meet the needs of the participant, in accordance with established medical practices, the regulatory guidelines and the policies and goals of this facility.
    • Provides information to participants and caregivers on prevention techniques and other ways to maintain or improve the general health and psychosocial status of the participant.
    • Participate in end-of-life care coordination.
    • Provide backup coverage to other GMWP nursing.
    • Complete all required documentation in a timely and accurate.
    • Protect privacy and maintain confidentiality of all company procedures and information about employees and participants.

    Education and Certification

    • Graduate of an accredited school of nursing; Bachelor of Science (BSN).
    • Wound care or treatment certification preferred.
    • Unencumbered California Registered Nurse (RN)License.
    • Current Basic Life Support (BLS) / First Aid Certification.

    Skills & Experience

    • Minimum of two years nursing experience in a clinical setting with a frail or elderly population.
    • Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
    • Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency.
    • Wound care or treatment certification.
    • Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
    • Experience and competency working with people from diverse backgrounds.
    • Commitment to unlocking the full potential of our most vulnerable seniors.
    • Bilingual Spanish-English preferred.

    Medical Clearance
