- Educate clients on their plan of care
- Coach family members on the clients plan of care and home safety
- Communicate client changes and provide an updated plan of care to the office team
- Graduate of an accredited nursing program and active RN license in OR
- Minimum of 1 years of nursing experience, ideally in home healthcare
- CPR certification, Criminal Background Check, Drug Screen
- Knowledge of state and federal regulations for PCSS
- Clinical proficiency in nursing specialty, strong communication skills and compassion
