    Wood County Hospital is accepting applications for an RN Care Coordinator.

    This position is r esponsible for conducting patient assessments and ensuring completion of referrals to applicable parties, high-risk screenings, and crisis interventions.

    Reviews patient records for medical necessity, level of care, length of stay, and authorization denials for inpatients and observation patients.

    Collaborates with the healthcare team to coordinate care according to evidence-based, approved guidelines. Develops and facilitates care plans throughout patient hospitalization and episode of care. Provide services which support optimal transitions of care and assists in the clinical appeal process.

    Job Duties:

    Shift Details:

    Hours:
    Full Time - 40 hours per week

    Shift: 7:30am-4:00pm


    Minimum Qualifications:

    Minimum Education:

    • Graduate of an accredited nursing program; bachelor's degree in nursing (BSN)preferred.

    Minimum Certification:

    • Licensed RN in the state of Ohio
    • BLS certifiedthrough the American Heart Association

    MinimumExperience:

    • 1-year previous experience in UR and Case Management.
    • 2 years of acute care nursingexperience required.
    • Experience utilizing InterQual and MCG preferred.
