Service Coordinator – Case Manager We are growing and looking for employees who are enthusiastic about working in a position that enhances the lives of seniors This position requires Spanish fluency Interview new participants and establish treatment goals in accordance with the Individual Plan of Care Complete assessment documentation and assist with arranging and monitoring community-based services The pay range for this position is $19 to $21 per hour Assess plan implement coordinate monitor and evaluate the services required to meet a participant's needs Participate in the completion of comprehensive assessments plans of care Maintain create service coordination notes from ongoing basis Inform refer participants available community services provide coordination medically related physical mental behavioral service needs participant Assist personal social family adjustment problems experienced Program Refer participant family appropriate community resources specialized counseling Coordinate if assistance other agencies acting coordinator services participant A minimum one year experience adults professional capacity degree certificate human services preferred Job Type Full-time Salary: 21.00 per hour Expected hours: 35 – 40 week Benefits Employee assistance program Paid time off Parental leave Professional development assistance Referral program Schedule Monday Friday Weekends needed License/Certification CPR Certification Preferred Work Location In person Edit jobOpenView public job page Job Type Full-time Pay: 21.00 per hour Language Spanish Required License/Certification CPR Certification Preferred Work Location In person
