    Job Description:

    Duties/Responsibilities:

    • The pharmacy technician plays a crucial role in medication reconciliation by:
    a. Collecting accurate medication information from the patient's residence and current pharmacy.

    b. Compiling and recording the patient's complete home medication list.

    c. Verifying information for accuracy and completeness.

    d. Documenting the patient's full home medication list, retail pharmacy details, and vaccination history when accessible.

    • Expected to execute most tasks independently following established departmental and hospital protocols.
    • Demonstrating essential skills to effectively engage with patients and families from diverse backgrounds and age groups.
    • Assisting in preparing irrigation and intravenous admixture solutions using aseptic techniques.
    • Dispensing unit doses, maintaining stock levels in various departments, and managing Automated Dispensing Machines (ADM).
    • Delivering unit doses, ADM refills, intravenous solutions, and required pharmaceutical supplies to the nursing units.
    • Labeling medications, which are subject to final verification by a pharmacist.
    • Preparing pre-packaged medications in commonly used quantities.
    • Supporting inventory control of supplies and medications, including receiving, shelving, and conducting cycle counts and monthly stock reviews for expired medications.
    • Cleaning pharmacy equipment, utensils, and work areas.

    Education/Experience Required:

    • Minimum age requirement is 18 years.
    • Proficiency in reading, writing, and comprehending English at a 12th-grade level, following instructions, and performing basic arithmetic calculations.
    • Successful completion of the Board of Pharmacy Technician Registration exam and all prerequisites of Pharmacy Technician Trainee.
    • Familiarity with basic IV admixture procedures and sterile techniques.
    • Strong aptitude in math, computer literacy, and communication.
    • Sound knowledge of medication names (brand and generic), common dosages and sig codes, as well as medical terminology.
    • Current PTCB certification or completion of 2 years of pharmacy school.
    • Minimum score of 85% on the Medication Reconciliation Technician Test.

    Registration/Certification:

    • Registration with the Massachusetts Board of Pharmacy as a Technician or Pharmacy Intern.
    • Certification by the PTCB (Pharmacy Technician Certification Board) or completion of 2 years of pharmacy school.
    • Maintenance of licensure from the Board of Pharmacy and fulfillment of continuing education obligations for PTCB.

    Physical/Environmental Requirements

    This role involves frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking, and lifting objects over 10 lbs. Occasional tasks may include climbing stairs, lifting heavy items, and sitting. Continual use of hearing, vision, and speech. Frequent use of smell and touch. Regular utilization of keyboards and telephones. Continuous focus and problem-solving.
