- Being a core member of the primary care team to see patients in assigned clinic(s).
- Providing Mental Health Home services, team support/consultation, and population health services.
- Offering brief interventions for mental health issues, crisis assessment, intervention, and more for patients seen by medical providers.
- Functioning as part of an interdisciplinary team, providing expertise, leadership, and consultation when needed.
- Engaging in Population Health strategies, panel management, and maintaining patient registries.
- Developing relationships with other social support providers for effective referrals.
- Providing care/services tailored to the developmental level of the population served.
- Must be a Licensed Social Worker (LICSW) or licensed to practice independently.
- Hold MN state licenses or eligible for licensing in Minnesota, and the ability to become a credentialed provider with insurance entities.
- At least 2 years of post-licensure experience.
- Experience in diagnostic assessments, evidence-based clinical services for children and adults, psychotherapy, family therapy, and more.
- Excellent interpersonal and organizational skills.
