- One year experience with psychiatric patients preferred
- Meet with patients and the interdisciplinary team to provide updates to family and community partners.
- Create safe and healthy discharge plans and coordinate specialized aftercare.
- Serve as point of contact for patients' wellbeing with members not on the care team.
- Gather and document information in the social service assessment and discharge plan.
- Collaborate with community providers and the patient's significant others to locate resources for discharge.
- Coordinate placement of patients to ensure that patients are released to the least restrictive environment possible, without endangering safety.
- Act as the primary liaison to families as well as the community resources, relaying information to the team in written form as well as verbal.
- Perform care consistent with the scope of licensure and in accordance with the facility's policies and procedures.
- Work effectively with fellow medical staff and treatment team members.
- Any other duties as assigned.
