Shriners Children's is the premier pediatric burn, orthopaedic, spinal cord injury, cleft lip and palate, and pediatric subspecialties medical center.
We have an opportunity for a Licensed Vocational Nurse (LVN) to join our Care Management team in our Galveston, Texas location.
Job Overview:
The LVN is responsible for planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care.
These services are provided either face to face or telephonically to facilitate the adherence to a treatment plan and optimize patient health.
Responsibilities:
Collaborates by working with patients, families, caregivers, and the health care team to communicate, problem solve, and share accountability for optimal outcomes.
Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services to accomplish goals.
Monitors progress toward the goals of the plan and ensures revisions in response to changes in the patients needs and condition.
Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.Ensures coordination with other Shriners Hospitals for Children locations to best meet the patient's needs.
Accountable for the achievement of optimal outcomes within the standards of practice.
Demonstrates professionalism by emulating the standards of practice of care management.
Qualifications:
Licensure and Certification
LVN License in state of practice required
Experience
1 year experience required
Bilingual English/Spanish preferred
