    Position: Licensed Practical Nurse (LPN)

    Reports To: Supervisor of Nursing

    LPN positions are non-exempt and eligible for overtime pay for hours worked in excess of 40 during a single calendar week (Monday - Sunday).

    Location: BX, Queens & Staten Island.

    Position Overview

    Under the direct clinical supervision of the Supervisor of Nursing or designee and the Residence Manager, the LPN is responsible for providing essential nursing care and administration to each person assigned who lives in a Catholic Guardian Services (CGS) residence. Services and supports are to be carried out in a manner that creates a homelike atmosphere that fosters and prioritizes the health and safety of each person served. The LPN works under the direction and supervision of a registered nurse, ensuring that all medical, nursing and nutrition services are carried out in a timely manner in accordance with DOH, OPWDD and agency requirements.

    Essential Job Functions: Under the Supervision of a Registered Nurse:
    • Carries out person-centered health related supports and services in a respectful and positive manner to assist with the achievement personal outcomes and life goals
    • Protects health, safety and well-being of each person served to the fullest extent possible, in accordance with the doctor's orders and approved plans of nursing and service plans.
    • Adheres to medical and nursing routine, urgent and emergency care procedures in accordance with DOH, OPWDD and CGS medical and nursing policies and protocols
    • Monitors personal health and immediately acts on any changes in individual health status and needs
    • Maintains individual health records and ensures all identified needs are addressed appropriately in a timely manner.
    • Maintains the calendar and DSP database for all individual routine and special health care services
    • Provides guidance and assistance with basic care, including training, medication administration, positioning and feeding
    • Provides basic nursing care, including the insertion of catheters
    • Explains, discusses, and listens to staff, families and people served about improving the health conditions of each person served.
    • Reviews MARS, and all medical protocols for signatures and follow up
    • Follows agency requirements for medication ordering, storage, inventory and disposal.
    • Acts in the capacity of health care advocate during medical appointments, inpatient hospitalization or rehabilitation stays and ensures that medical information is obtained, clearly documented and communicated to the clinical team, person, family and following all medical visits.
    • Carries out all interventions, procedures and notifications, including but not limited to medical, behavioral and nursing, in accordance with the service plan, agency requirements and OPWDD regulations.
    • Promotes and practices universal precautions and "safe patient handling" in carrying out all health care related responsibilities.
    • Attends to personal hygiene needs of each person (i.e. toileting, bathing and changing as per the service plan and as the situation warrants).
    • Ensures that all measures for health, safety and well-being are maintained at the highest level by maintaining a clean, safe, personalized, and homelike atmosphere.
    • Prepares complete, concise written reports including but not limited to incident reports, progress notes and completes all required documentation in an accurate and timely manner.
    • Ensures actions and services are conducted in a manner that uphold the right to privacy, support informed decision-making and prevent abuse and neglect.
    • Transports individuals in agency vehicles or using public transportation in a safe and secure manner.
    • Observes and assists team with carrying out diet and nutrition planning, preparation and implementation in accordance with each person's needs and orders.
    • Fully participates in all meetings and trainings as required.
    • Assists RN with overseeing, monitoring, administering and properly documenting prescribed medications/treatments to persons served in accordance with agency protocol and state and federal regulatory requirements.
    • Prevents, recognizes and immediately reports all incidents, including allegations of abuse and neglect, serious reportable incidents and exceptional occurrences in accordance with agency policy and OPWDD (Office for People with Developmental Disabilities) and Justice Center requirements.
    • Activates the agency emergency notification protocol and summons police, fire, ambulance or other emergency personnel when warranted.
    • Assists staff with immediately acting to prevent or redirect behavior that is threatening or dangerous to the person served or others and exercises appropriate interventions in accordance with OPWDD regulations, and/or SCIP-R guidelines, the person-centered plan of care and level of training.
    • Communicates in a respectful manner with all people including the people served, families and advocates, agency staff and members of the broader community.
    • Adheres to the staff Code of Ethics and all confidentiality guidelines in accordance with HIPAA requirements and agency policy.
    • Meets routinely with the Residence RN for supervision, support and guidance.
    Minimum Job Qualifications
    • NYS License as a Licensed Practical Nurse must be kept current and in good standing at all times.
    • High School Diploma or GED
    • Minimum six months experience working in the field of IDD as a nurse.
    • Valid New York State Driver's License that is maintained in good standing.
    • Must transport individuals in agency vehicles.
    Skills and Abilities Required
    • Ability to read, write, speak and understand English and follow instructions
    • Good oral and written communication skills
    • Ability to use computers or smart devices to carry out responsibilities.
    • Good interpersonal skills necessary to interact effectively with all parties.
    • Ability to analyze problems and determine corrective measures
    • Complete all required trainings and maintain all required certifications (including First Aid and CPR, SCIP-R, eMAR). eMAR certification shall be completed within six months of hire.
    Physical and Emotional Abilities
    • Ability to lift at least 30 pounds or more
    • Daily standing, bending and lifting
    • Must be able to stand for at least eight hours at a time
