Patient Care Coordinator - Home Health
JOB SUMMARY
As Patient Care Coordinator you will be responsible for coordinating patient care between the agency, physician office and other healthcare institutions; provide support to the office staff with various tasks such as answering phones, processing medical records, ordering supplies, coordinating staff schedules, contacting patients to verify demographics, facilitating a care transition.
QUALIFICATIONS
Relevant experience in home health and similar
Knowledge of basic medical terminologies
Great task and priority management
Great communication skills
Good computer and telephone systems knowledge
