The Dialysis Social Worker (LCSW) plays a crucial role in evaluating and addressing the emotional needs of our dialysis patients to enhance outcomes and enhance their quality of life. Schedule: Part-time, approximately 6-10 hours per week. Responsibilities What You Can Expect: - Provide emotional, financial, and lifestyle support to patients dealing with chronic kidney disease - Assist patients in accessing community resources for nutritional, emotional, transportation, and housing needs - Advocate for patients to access healthcare services and other benefits - Educate patients and families on treatment options for end-stage renal disease, such as dialysis, transplant, and medical care - Support patients in making informed decisions about their treatment by comparing different options - Organize translation services for patients and their caregivers - Evaluate the patient's family and support system, making referrals for additional services when required - Offer counseling services to help patients and families cope with kidney failure and dialysis, adhere to treatment plans, and achieve rehabilitation goals - Develop personalized care plans with the care team based on a psychosocial assessment to enhance the patient's quality of life - Collaborate with the care team to reassess needs regularly and adjust care plans when necessary - Educate other team members on the emotional aspects related to dialysis - Champion for patients at the clinic, in the community, and at state/federal levels - Act as a liaison with the ESRD Network - Participate in quality improvement initiatives driven by data to improve outcomes
