    The medical social worker is responsible for the implementation of standards of care for medical social work services and as defined by the state Medical Social Work Practice Act.

    What your responsibilities will be:


    Conduct initial psychosocial and initial bereavement assessment of patient/family/caregiver in a thorough, accurate, compassionate, within five days of admission to hospice.

    Function as a support person for the patient/family/caregiver on an ongoing basis.

    Function as expert and facilitator of obtaining community resources to meet the needs of patients and families, including financial assistance.

    Assist families with referrals to community resources when indicated.
    Assist family/patient with advanced directives.
    Assist family/caregiver in placement of patient in a facility for basic care, when appropriate.

    Complete and maintain all patient records at patient's residence and submit accurate and relevant notes regarding patient's condition and care given.

    Attend and actively participate in team meetings, staff meetings and in-services, and contributes positively to patient care planning
    Conduct pre-bereavement and bereavement visits or contacts and/or support groups as appropriate.
    Work with other members of the interdisciplinary team.
    Provide and maintain a safe environment for the patient,
    Assist patient/family/caregiver and other team members in providing continuity of care.
    Participates in evening/ weekend on call rotation, conducting on call services in a clinically competent and responsive manner.

    Maintain clinical records, statistics, reports and records for purpose of evaluation and reporting of agency activities as prescribed and in compliance with local, state and federal laws.

    Maintain caseload and deliver care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies.
    Assure for compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
    Maintain all appropriate state licensing requirements.

    Actively participate in staff meeting meetings, department meetings, team meetings, briefings, in- services, committees and other related activities as needed.

    Use supplies and equipment safely and efficiently.
    Meet or exceed delivery of company services standards in a consistent fashion.
    Interacts with staff in a positive and motivational fashion supporting the company's mission.
    Conducts all business activities in a professional and ethical manner.
    Create and maintain a professional atmosphere.
    Meet with patient and family/caregiver to present hospice program.
    Obtain signed consents by POA for HC and patient if patient is alert and oriented.

    Master's degree from an accredited school by the council on social work education required in states as specified. BSW is acceptable under MSW supervision.
    Licensure and/or certification as required by the state.
    Minimum of one year social service experience in the health care setting or social services agency.
    Volunteer coordinating experience a plus
    Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, erc.)
    Compliance with accepted professional standards and practices.
    Understands Hospice philosophy, principles of death/dying.
    Ability to work in an interdisciplinary setting.
    Physical exam in accordance with state law requirements.

    Valid driver's license and an automobile that is insured in accordance with state and/ or company requirements and is in good working order.

    Satisfactory references from employers and /or professional peers.
    Satisfactory criminal background check.
    Compliance with acceptable professional standards and practices.
    Ability to work within an interdisciplinary setting with little supervision.
    Demonstrate excellent observation, judgment and communication skills. Ability to provide good written documentation in a timely manner.
    Possesses and maintains current CPR certification.
