- Assist with home health referrals and clinician scheduling
- Provide telephone and clerical support
- Assist with clinician documentation review
- Manage verbal physician orders and patient lab results
- Assist with complete medical records requests
- Participates in weekend on call rotation, answering and triage phone calls
- Create, maintain, and update the clinician's schedules according to authorizations and patient needs.
- MUST BE AN LPN with a current/active MO license in good standing
- Previous Home Health experience preferred.
- Excellent organizational and time management skills.
- Excellent communication and customer service skills
- Ability to manage multiple concurrent tasks and prioritize assignments.
- Ability to communicate effectively with multidisciplinary team of clinicians, clerical staff, management, patients/families, and referral sources.

Job Summary:
