    Description
    SUPERVISOR: Service Support Manager HOURS: M-F 7-5, Some Sat. 9-12, must be flexible

    CLASSIFICATION: Full time/ Hourly/Non-Exempt TRAVEL / LOCATION: multiple locations

    QUALIFICATIONS:
    • Demonstrated ability and familiarity with tools, technology, and systems found within health care environments (such as Basic Computer Skills, Electronic Medical Records, Experience with Medent EMR a plus).
    • At least 1 year of experience in working in a medical office
    • Ability to demonstrate and model professionalism, candor and care of patients, visitors and organizational partners
    JOB SUMMARY: The Patient Navigator Staff member will work under the direction of the office manager or site manager in support of JRCHC providers to help guide the patient through the healthcare system in effort to overcome barriers that hinder patients from receiving treatment. Patient Navigators support front office staff to attend to and assess patients' needs while coordinating and organizing appointments and documentation to facilitate the smooth running of JRCHC, supporting the delivery of quality patient care.

    Essential Job Duties

    PATIENT INTAKE AND CARE
    • Work with JRCHC providers by scheduling and confirm patient diagnostic appointments, surgeries, and other specialty appointments that may not be a direct service of JRCHC
    • Provide customer and personal services, including customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
    • Practice high level of ethical behavior while interacting with patients and appropriate staff
    • Practice cultural sensitivity while working with JRCHC patients and staff
    • Assist with front office receptionist responsibilities as needed
    ADMINISTRATION
    • Enter accurate insurance information and verify insurance availability
    • Receive and route messages or documents to appropriate staff
    • Maintain and transmit correspondence or medical records, by mail, email or fax
    • Protects patients' rights by maintain confidentiality of personal and financial information
    PHYSICAL REQUIREMENTS: Must be able to sit or stand for long periods of time. Exert up to 50lbs. pushing, pulling, or lifting. Walking, climbing stairs required. Able to talk/hear and communicate with others with or without reasonable accommodation.
