Job summary
The Dementia Program Activities Coordinator will work with dementia patients receiving hospice care at home and in facilities to find activities that engage the interest of persons with Alzheimer's disease and other dementias.Responsibilities
- Speaks with family members/caregiver, facility staff and Haven team to identify areas of interest and engagement opportunities for patient.
- Demonstrates advanced knowledge of providing engagement and care to patients with dementia, as well as available community resources for patients and caregivers.
