- Providing professional nursing services under the guidance of a RN in compliance with relevant regulations, healthcare standards, laws, and company policies.
- Delivering skilled nursing interventions for patient/client treatment, rehabilitation, and preventive care. Implementing a comprehensive approach in providing problem-specific therapeutic interventions, educational activities, and training as per the care plan.
- Applying expertise and skills based on accepted clinical practice standards to facilitate issue resolution and achieve personalized patient objectives and outcomes.
- Collaborating with the RN Supervisor for necessary adjustments in the Treatment Plan. Following verbal orders from physicians as permitted and conveying these orders to the RN Supervisor.
- Employing a systematic, individualized approach focused on goals and outcomes in executing the nursing care plan.
- Maintaining communication with patients, physicians, clinical managers, and other healthcare team members promptly regarding patient/family requirements and status changes. Engaging in care coordination and discharge planning when required.
- Upholding the highest levels of professional conduct concerning confidential information. Sharing information only when authorized and in the best interest of the patient.
- Ensuring adherence to professional standards in the availability and provision of supplies, materials, and equipment essential for the effective implementation of the care plan.
- Generating, submitting, and managing documentation as per the Company's requirements. Recording visit/shift notes on the same day services are provided.
- Graduate of a certified Licensed Practical Nursing Program or Vocational Nursing School.
- Current nursing license in the state of practice.
- Valid driver’s license, auto insurance, and dependable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
