    Salary:
    $75,000.00 $85,000.00 per year, $2000 hiring bonus ($1000 at 30 days of hire and $1000 at 6 months of hire)
    This is a full time position.
    Duties

    As a Licensed Care Professional, you will work across Wilders Customized Living Services provided at two Saint Paul Public Housing sites (Ravoux and Hamline Hi-Rises).

    Together with Wilders Healthy Aging & Caregiving Services, you will join a collaborative team that supports the health and independence of adults to age and be cared for in the community, critical to Wilders mission of improving lives today and for generations to come

    In your role, you will provide professional nursing services on a person-centered basis to older adults and adults with chronic conditions.

    You will provide supervision and work direction to licensed health care staff and training for health care technicians, assist providers and others to manage healthcare needs, assess clients, and monitor medication needs of clients.

    Communicating regularly with an interdisciplinary team and completing documentation as directed is also essential.

    Job Qualifications

    Associates degree required (Bachelors preferred), with 3-5 years of relevant nursing experience, and at least 1 year experience in training, leading, and supervision of direct service staff.

    MN Nursing Licensure. Experience in geriatric needs and issues, home health, customized living or long term care setting preferred.

    Must be self-directed and detail oriented with the ability to differentiate between independent nursing decisions and those requiring delegation from physician.

    Strong organizational skills, problem-solving ability, active listening and excellent communication skills. Manage time well and perform a variety of tasks. Flexible and able to work with a diverse team and adapt to meet the needs of a diverse clientele. Proficiency in Microsoft Office, e-mail and other software as required. Ability to compose and edit text. Ability to use good judgment in a variety of challenging situations. Ability to work under pressure and meet tight deadlines. Knowledge of and/or ability to become proficient in an electronic health records system.
