What You Will Do

• Deliver outreach services to mentally ill individuals in the community
• Perform psychosocial evaluations and assess mental health service needs
• Create and execute short-term service plans in collaboration with a team
• Respond promptly to dispatch calls and ensure seamless program operations
• Provide comprehensive care from counseling to treatment referrals
• Prepare and maintain case records
• Encourage resistant clients to accept mental health services
• Provide clinical supervision when necessary

• Current registration as a Licensed Master Social Worker in New York State
• Valid driver's license may be required
• Minimum one year of social work experience in healthcare settings
• Master's degree in social work from an accredited institution
