    Job Title:

    Physical Therapist (PRN) - Post-Acute Rehab Solutions

    About the Role:

    We are seeking a dedicated and compassionate Physical Therapist to provide high-quality patient care in post-acute rehabilitation settings. As a key member of our team, you will play a vital role in helping patients achieve their maximum potential and quality of life.

    Key Responsibilities:

    • Patient Assessment and Goal Setting: Identify and meet patients' goals and needs through comprehensive assessments and collaborative goal setting.
    • Treatment Planning and Intervention: Develop and implement personalized treatment plans that promote cost-effective improvements in patients' mobility and functionality.
    • Care Coordination and Collaboration: Consult with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
    • Education and Community Outreach: Provide education and resources to patients and their families to promote healthy lifestyles and independence.
    • Documentation and Communication: Accurately document patient care services and communicate effectively with interdisciplinary teams to ensure continuity of care.

    Requirements:

    • Current licensure as a physical therapist in state/region
    • Graduate degree or greater in physical therapy from an accredited program
    • Proven experience working in post-acute rehabilitation settings
    • Excellent communication and interpersonal skills
    • Ability to work independently and as part of a team

    Salary Range:

    $65,000 - $90,000 per year, depending on experience
