Job summary
The Social Worker LCSW is fully engaged in providing quality/no harm, customer service and stewardship by utilizing advanced psychosocial skills to facilitate the coordination of patient care through assessment, planning, intervention and evaluation.
The Social Worker LCSW uses current knowledge to effectively evaluate, promote and utilize hospital and community resources and services to achieve optimal outcomes, serving as a role model for coordination of psychosocial services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers.
- Collaboratively determines desired discharge goals and communicates all discharge plans and options to patient/family physicians staff members.
- Provides counseling researches placement alternatives for substance abuse mental health homeless indigent patients.
- Facilitates participates in transfer Baker Act Marchman placements mental health facilities Provides therapeutic interventions with patients families needed Completes mandatory reporting processes Facilitates referrals post-discharge psychosocial therapeutic needs In crisis situation where safety of the patient others immediate jeopardy LCSW can Baker Act a patient Mentors social work students local universities internship department Provides clinical supervision social workers pursuing LCSW Identifies systemprocess breakdowns improvement opportunities documenting Performance Improvement Plan Intervenes recommends appropriate referrals resolve system barriers patient progression collaborating disciplines directed Communicates avoidable delays RN Case Manager collaboration documentation MIDAS Ensures compliance Federal state regulatory standards care identifying risk management quality issues intervening referring indicated
