Long Term Care (LTC) - Licensed Practical Nurse (LPN):

• Explore the Beautiful Finger Lakes region in Upstate NY
• 2 hours from Niagara Falls
• 13-week contract, possible extensions

Licensed Practical Nurse (LPN) Position Details:

• 13 week Contract with high probability to extend
• May start dates
• Days or Nights available

Licensed Practical Nurse (LPN) Job Description:

• Collects data relevant to resident's physical and psychosocial condition on an ongoing basis and communicates finding to RN and other members of the Interdisciplinary Team.
• Contributes to the creation of a developmentally age appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes involving the resident/family and healthcare team in this process while promoting a healthy, safe environment.
• Recommends revisions to the plan of care, after evaluation of resident outcomes and indicates the need for referrals to ensure all aspects of the resident's care have been addressed.

Qualifications:

• At least 1 year of LPN Experience; Long Term Care Experience preferred
• Active LPN NY License
• BLS (AHA) required
• Pay:
• $1,540 - $1,590 Total on 40 hours
• LOCAL and TRAVEL PAY options available
