Role Description
This is a full-time hybrid Care Coordinator role with Soran Health located in Miami, FL, with flexibility for remote work. As a Care Coordinator, you will work with healthcare professionals to manage care for patients, perform health assessments, and navigate patients to the proper resources. You will also need to develop and implement individual care plans and schedule appointments for patients.
