    Are you looking for a rewarding part-time position that allows you to make a meaningful impact in the healthcare community? Centered Health is excited to announce an opening for a Continuing Care Specialist


    In this fully remote role, you'll have the flexibility to work 20 hours per week, Monday to Friday, with compensation of $14.00 per hour.

    Additionally, our generous production-based incentive plan offers you the chance to enhance your income based on your contributions. You will work Pacific Time hours.

    As a Continuing Care Specialist, you will play a vital role in cultivating and maintaining strong, trust-based relationships with our external treatment providers.

    Your efforts will directly contribute to the continuity of care for our clients, ensuring they receive the best possible support during their treatment journey and beyond.

    To thrive in this role, you should possess strong interpersonal skills and a warm communication style. Native English speakers with excellent written and verbal communication skills are preferred.

    You should be self-motivated, capable of working independently in a remote setting, and comfortable using electronic health records and CRM platforms, such as Salesforce.

    Familiarity with HIPAA regulations and healthcare communication practices is essential, along with the ability to multitask and manage multiple provider relationships with meticulous attention to detail.

    Being accountable and proactive in your approach will be key to your success.

    As a Continuing Care Specialist, your responsibilities will include:
    Maintaining active, professional relationships with external treatment providers (e.g., therapists, psychiatrists, educational consultants).
    Conducting routine post-discharge check-ins to ensure clients are progressing and evaluating provider suitability for future referrals.

    Pursuing new referrals from providers who have previously received clients from Centered Health, leveraging successful outcomes for ongoing referral streams.

    Documenting all treatment provider communication in Salesforce, tracking engagement and satisfaction.
    Collaborating with clinical and admissions teams to gather and communicate post-discharge information in a HIPAA-compliant manner.
    Organizing regular touchpoints with providers, including check-ins, follow-up emails, and thank-you notes.
    Tracking treatment provider satisfaction and assisting with reporting for quality improvement and business development purposes.
