Role Description:
The main goal of a Primary Care Physician at Oak Street Health is to offer fair and efficient value-based healthcare to local Medicare patient groups at our innovative network of neighborhood primary care centers.
Our Primary Care Physicians function at a single medical facility backed by large care teams to allow our providers to concentrate on providing higher quality care rather than a high volume of services.

We are seeking physicians who are genuinely driven to offer top-notch care to older individuals. Recent graduates are welcome to submit their applications.Outstanding candidates may also be considered for a Center Medical Director role.
Responsibilities:
- Evaluate and diagnose patients at our local clinics
- Supervise, manage, and provide primary care
- Prescribe and administer medications and pharmaceutical treatments
- Manage Patient Electronic Medical Record data through canopy and greenway
- Cooperate with regional and central leadership to achieve health quality objectives
- Collaborate with Practice Managers to lead and supervise the center care team
- Any other duties as assigned
Required Qualifications:
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Certification in Internal Medicine or Family Medicine (Or eligible for certification)
- Valid, unrestricted State License
- Active DEA license
- Authorization to work in the US
- Individual who embodies the essence of being "Oaky"
Preferred Qualifications:
- Specialized training in Geriatrics
- Experience in treating Geriatric populations
- Experience working in a Value-Based Healthcare Model
- History in a collaborative environment to ensure positive health results
- Experience in outpatient primary care settings
- Proficiency in multiple languages where applicable
