- Coordinate with community care teams to ensure comprehensive support is available at each point of care.
- Link unsheltered individuals identified through EDs and outreach programs to appropriate CAN (Coordinated Access Network) resources and outreach support.
- Ensure linkage of clients to nursing services, case management, mental health treatment, substance use treatment, primary care, and specialty medical services.
- Establish routine engagement protocols with ED staff to facilitate identification and referral of high-utilizing unsheltered individuals.
- Conducts Medical Assessments (a client self-report on medical history), LOCUS, Co-occurring Screens and Functional Assessments.
- Support individuals with participating in their medical appointments or procedures, obtaining updates vitals quarterly, and assisting clients with obtaining lab work as ordered.
- Collaborates with individuals to create individualized, person centered treatment plans and documents clients progress during treatment.
- Provides individual and group education around self-care, medical conditions, mental illness, and medications..
- Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions
- Support individual with linking and engaging with natural (family, friends) or community supports
- Develop crisis plans for clients and work with other staff to respond to crisis situations as needed
- Collaborating with Primary Care, Hospitals/ER's, Visiting Nurses and Specialty Medical Providers in the community.
- Provide education, support and coaching around skills of coordinating self-care and medical/behavioral health care needs.
- Provide skills teaching, such as assisting clients in obtaining/ maintaining housing and utilities; learning how to use transportation services, planning and preparing nutritional meals including diet and nutrition, Diabetes education, illness prevention, engage and communicate with family and friends, be safe in the community, care for children and other family members, and solve everyday problems that arise.

ABOUT THE PROGRAM: HOUSING INNOVATION
Education: Graduate of school of Nursing with LPN licensure.
Experience: Experience with patients with complex medical and behavioral health issues.
Licensure/Certification/Registration: Licensed Practical Nurse in State of CT. CPR/First Aid Certification within 6 months of hire. Valid driver's license.

$22.72/hr minimum- $34.08 maximum, dependent upon experience.
*Actual rates are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
