    Position Title:
    Clinical Director, Health Homes Care Coordination Program


    The Clinical Director, under the direction of the Assistant Vice President of Operations, will be responsible for overseeing the daily operation and overall management of the high-risk patient Health Home (HH) programs, including HARP/Home Community Based Services, Assistant Outpatient Treatment and Health Home Plus.


    Provide administrative and clinical supervision to managerial staff and indirectly supervises care coordination staff, ensuring the provision of strong clinical programming to all participants and the professional development of staff.

    Oversee the day-to-day operations, including assignment of cases, reviewing comprehensive assessments, care plans and various administrative tasks to ensure the provision of comprehensive, trauma informed, culturally responsible services that addresses issues such as intimate partner violence, substance use, trauma, mental and behavioral health, etc.

    Monitors overall program productivity, KPIs and quality indicators.
    Work in collaboration with other onsite managerial staff to facilitate referrals into the high-risk programs.
    Identify best clinical practices, new interventions and implement them with the department, including identifying clinical models for implementation.
    Participate in and monitors quality improvement activities, including chart reviews, implementing corrective action plans and process improvement.
    Represent the agency at community conferences and meetings.
    Ensure compliance with Federal, State, and City regulations as they relate to Health Home enrollment process.
    A master's degree in social work, psychology or related social services field.
    Proven experience in leadership or management role within a case management, mental health, substance use or similar programs.
    Strong knowledge of mental health practices, treatment modalities and evidence-based interventions.
    Familiarity with relevant laws, regulations, and ethical guidelines governing mental health services (HIPAA, Medicaid/Medicare, etc.)

    Proficiency in electronic health record (EHR) systems and other clinic management software
    Commitment to providing high-quality, client-centered care and promoting mental health awareness.


    Demonstrated comfort in use of common software, including Microsoft Office (i.e., word, PowerPoint, excel etc.), and Adobe Creative Suite, and Learning Management Software and providing training in virtual environments (i.e., Familiarity and/or experience with working with people with mental health, substance use, and chronic conditions.

    Administrative writing and reporting skills.

    Bilingual, fluent Spanish strongly preferred.
