- Assessment/Diagnosis - Performs initial ongoing and functional health status assessment as applicable to the population or individual.
- Outcomes Identification/Planning - Documents planned nursing interventions to achieve outcomes appropriate to patient needs.
- Implementation/Evaluation - Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals.
