    FAMILY ADVOCATE PARTNER FOR FAMILY SUPPORT PROGRAM IN PHILADELPHIA
    Hourly Rate Range: $18-$21 an hour depending on education and experience
    The Family Support Program is an evidence-based prevention program that has shown incredible outcomes for families receiving these services that will now be offered in Philadelphia through Gemma Services. The Family Support Program is a home visiting model that will be offered to families with young children in the Philadelphia area seeking assistance.
    The Family Advocate Partner (FAP) plays a critical role in bridging the gap between families and the services we offer.

    Job Specifics: With compassion, and practical support, the FAP helps families engage meaningfully with the program and navigate challenges with confidence. The (FAP) provides hope, builds connections and linkages, and encourages parents to utilize their voice to be part of a collaborative problem-solving process. The (FAP) will be integral to the Family Support team to support families in the program with the following below:
    • Support families in accessing services, navigating systems, and attending appointments
    • Assist with home-based skill practice and provide child engagement during Family Support sessions
    • Provide outreach support, attend community events, and assist with family enrollment
    • Facilitate Parent Café sessions and promote peer support
    • Build relationships with community partners and daycares
    • Conduct home visits and connect families to community resources
    • Complete timely and accurate documentation
    • Participate in required trainings, meetings, and performance improvement activities
    • Ensure compliance with agency policies and regulatory standards
    This is a hybrid role: Monday - Friday, full-time position that is based out of our Preheim Center in Southwest Philadelphia. The position works in the field throughout the city of Philadelphia and must be able (and interested) to travel daily and independently. The schedule varies and must be approved by the supervisor.
    This position requires lived experience in a child-serving system as a parent or child which includes mental or behavioral health, child-welfare, or juvenile-justice services. A minimum of 1 year of experience in childcare, preferably within a Keystone STARS organization. A high school diploma is required, bachelor's degree preferred. Familiar with services for children in the managed care system. Must have reliable transportation and a valid driver's license.
