Solstice Hospice & Palliative Care is a locally owned hospice committed to providing compassionate, high-quality, patient-centered end-of-life care. We are currently seeking a Full-Time Hospice Case Manager to join our dedicated clinical team.
Responsibilities
- Manage and coordinate a caseload of hospice patients in home, facility, and assisted living settings ,
- Perform comprehensive nursing assessments and develop individualized plans of care ,
- Provide skilled nursing visits, symptom management, and patient/family education ,
