Palliative Care Social Worker at St. Luke's Health System in Meridian, Idaho, United States Job Description Overview St. Luke's Health System in Nampa is seeking a Social Worker to join our Palliative team.Our Inpatient Palliative Care Social Worker will work, in tandem, with a provider to provide consults as needed at our Nampalocation.
Palliative care is specialized medical care for people living with a serious, progressive, often life limiting illness.This type of care is focused on providing relief from the symptoms and stress of the illness, to improve quality of life for both the patient and the family.
Examples of serious illness often seen by a palliative care team include all types of cancer, end stage organ disease (e.g., heart, lung, liver, kidney), dementia, Parkinson's disease, and other progressive neurological disorders.
Social workers are integral members of the interdisciplinary palliative care team, addressing psychological and social needs to support patients living with serious illness, and their families.
Because of their dedicated training, palliative care social workers play a critical role on the team. Responsibilities + Completes psychosocial assessments and plans of care for identified patient populations. + Identify and provide interventions and goals as indicated by the plan of care. + Provides crisis intervention services to patients, families, and staff.+ Provides consultation to physicians, healthcare professionals, or community partners in understanding psychiatric, behavioral, social, emotional, and cultural factors related to plan of care for the patient.
+ Consults with staff regarding clinical situations; assists inter-disciplinary team in dealing with complex, high-risk or difficult psychosocial situations. + Serves as a member of a multi-disciplinary team and is a liaison with community resources. + Provides community resource information and referrals. + May provide individual, group, or family therapy. + May provide supervision, leadership, coaching, and/or mentoring to other clinical staff including co-signature of documentation. May provide counter signature of other's clinical practice. + If state certified, may provide clinical oversight to staff towards advanced licensure. + May participate in Third Party billing. + Other duties and responsibilities as assigned.Qualifications + Education:
