Unit Overview: ?
SICU: 29 bed unit. Open model. Usual census is about 20 patients. Will take surgical patients, and will take overflow for Stroke if Neuro ICU is full. Will fill up during the winter months. They will also take other overflow.
7th floor: 30 beds; 2 units (12 beds MICU: all things medical; have longer LOS; 18 beds: CCU -> all things cardiac). Units take overflow. Average Daily Census: 10 in MICU, 14 in CCU.
CVICU: Takes CV surgery. This is covered.
Neuro ICU: 6 beds. Post stroke, covered by Neurologists
There are other stepdown beds that are available (about 6 beds or so). .
Proposed coverage model: 2 daytime intensivists covering 25-27 patients total. The intensivists will see the sickest 12-13 patients in the SICU and MICU/CCU. They will full manage those patients, run MDRs, and then transfer to the HM docs once they get better.
There are progressive care units. There is a respiratory step down (no vents) and a cardiac step down unit.
Current protocols: sepsis orderset, therapist-driven weaning protocol, TTM orderset, stroke orderset, they have physician-specific sedation protocol for vents. They also have MDRs currently.
Procedures: Central lines are done by IR or at bedside. At night, the lines are done in ER as well. RT and Anesthesia does intubate. At night, IR/Hospitalist/Fam Med residents can do lines. There is a CVC cart. They have US. OR has a Glidescope.
EMR: Cerner
RRT/Code Blue: Housewide medical response team from the nursing team. Residents respond to codes, and then the Hospitalists/PMD respond. No expectation that the Intensivists will respond to RRT/Codes.
