With over 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community.

We've got a great opportunity for an Outpatient Clinician to join our new integrated family care clinic, Health Solutions Family Medicine at Perry Hall. This position will be integrated into our Health Solutions Family Medicine location at 41 Montebello Rd. in Pueblo, CO.

What You'll Be Doing

As the Clinician you would assess and provide just-right therapies for our clients in need. You would coordinate care, actively liaise with internal and external partners, and serve as an important part of a multidisciplinary team. You would employ excellent communication and documentations skills on an ongoing basis to assure that clients receive the very best care.

RESPONSIBILITIES: As an Outpatient Clinician under the supervision of the Director and Supervisor you will:

  • Conduct screenings, intake assessments, and facilitating collaborative treatment planning; to include substance abuse/mental health/co-occurring assessments
  • Provide individual, couples/family, and/or group services
  • Provide case management services
  • Keep accurate, complete, and up-to-date records with 100% compliance with Center standards
  • Maintains monthly Health Solutions productivity expectations.
  • Maintain competency and proficiency with the electronic health record
  • Work with community agencies
  • Performing other job-related responsibilities as assigned
  • Will be required to cross train in other duties as allowable by licensure

Physical requirements

  • Requires standing, sitting, driving, and walking for prolonged periods of time
  • Occasionally lifts and carries items up to 20 pounds
  • Frequently exposed to stressful or emergent situations
  • Frequently operates a computer and other office equipment to record health data
  • Constantly communicates with clients and others
  • Occasional exposure to communicable diseases and bodily fluids
  • Occasionally drives between Health Solutions facilities or for other assigned duties
  • Requires occasional driving in adverse weather conditions

Qualifications

What we're looking For—The Must-Haves

  • Master's degree in social work, clinical psychology, or a closely related field
  • Current Colorado professional license (e.g., LPC, LCSW, LMFT)
  • Experience working successfully with a multi-disciplinary team, especially in an integrated primary care setting
  • Ability to work in a fast-paced primary integrated healthcare practice
  • Experience providing clinical services to patients of all ages
  • Experience and comfort working with a wide range of behavioral health conditions and presentations to include substance use disorders
  • Experience working with severe and persistently mentally ill adults, and experience with various behavioral health diagnoses including substance use
  • Good communication skills, both verbal and written
  • Exceptional time management skills
  • Flexibility and ability to easily adapt to change
  • Well-developed documentation skills
  • Computer proficiency
  • Valid Colorado driver's license, and access to reliable transportation during work hours
  • CPR/First Aid

What we'd Like to See in you—the Nice-to-Haves

  • Mental Health Assessment skills and familiarity with the DSM are preferred
  • Certified or Licensed Addiction Counselor preferred
  • Experience completing at-risk assessments and formulating appropriate treatments plans
  • Spanish language skills
