    Job Description

    We seek an independent and adaptable RN who promotes teamwork, demonstrates resilience, and provides excellent patient care. This leader will positively impact our patients and team in a fast-paced home healthcare and hospice environment.

    Our RNs implement the nursing process in the home, allowing patients to receive care where they feel most comfortable. The ideal candidate will be a skilled advocate with effective communication and interpersonal skills, willing to collaborate on a responsive team of professionals dedicated to providing high-quality care.

    Key Responsibilities:

    1. Implement the nursing process in the home care setting.
    2. Utilize critical and creative thinking to improve patient health status.
    3. Work collaboratively with patients and healthcare professionals to develop and implement personalized care plans.

    Qualifications

    To be successful in this role, you must have:

    • A current Minnesota RN license.
    • The ability to obtain American Heart Association BLS certification within 30 days of hire.
    • A valid Minnesota driver's license.
    • Previous experience in Home Care and/or Hospice (preferred).
