    CAMBA's Brooklyn 3D (Drive Down Diabetes) is a diabetes prevention and self-management program designed to address the widespread prevalence of diabetes and pre-diabetes in low-income communities of color in Brooklyn.

    Brooklyn 3D provides culturally relevant diabetes self-management and counseling to Brooklyn residents with pre-diabetes, type 2 diabetes, and gestational diabetes.

    B3D also provides personal training consultations via Zoom; care coordination with medical providers; and linkage to resources addressing key determinants of health, such as food insecurity and housing.

    Nutrition Assistant Does:

    • Conduct comprehensive onsite and virtual diabetes knowledge and nutritional assessments, service plans and reassessments with clients. Conduct diabetes self-management education sessions, including glucose self-monitoring. Coach clients on healthy, culturally relevant meal preparation. With clients' and/or client families' agreement, arrange for medical, psychiatric, educational and other tests and examinations that may disclose causes of client difficulties and indicate remedial measures. Coordinate with personal trainer about client fitness needs. Coordinate with other providers, including primary care provider. Assist clients in attaining their goals by identifying and locating community resources for clients and by making referrals to appropriate services both within and outside CAMBA. Follow-up with clients and with referral organizations regarding client contact and progress with referral organization. Monitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or office visits, and document via progress notes. Provide all required information for weekly/monthly/quarterly/annual reports.
    • Recommend closing of cases in which clients have:
    (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area).+ Input client data and client progress information into automated database.+ Reach out and market the program to the community in order to recruit clients.


    Minimum Education/Experience Required:

    • BA or BS in Nutrition or a related field Experience working with diabetes Bilingual English/Spanish Good written & verbal communication, and interpersonal skills. Proficiency in Outlook, Word, and Excel. Computer literacy in other Microsoft Office Suite products.

    Other Requirements:

    • Certified Diabetes Care and Education Specialist preferred

    Compensation:
    $50,000 annuallyWhen salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate.

    This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.


    Status:
    Full-time (35 hours per week)
